A cranial prosthesis is a medical term often used for a wig or hair replacement piece prescribed for hair loss related to a medical condition or treatment. Some insurance companies may use this term when reviewing claims for reimbursement.

When applicable, Locks and Tops can provide an itemized invoice using the term “cranial prosthesis” and include supporting details such as the product description, purchase amount, customization fees, business information, and HCPCS code A9282.

Please note that insurance coverage varies by plan. Locks and Tops cannot guarantee reimbursement, but we are happy to provide documentation that may help you submit a claim to your insurance company.

Before You Purchase

Before purchasing a wig or topper for medical hair loss, we recommend contacting your insurance company and asking about your coverage.

Helpful questions to ask include:

  • Is HCPCS code A9282 covered under my plan?
  • Is a prescription required?
  • Is a letter of medical necessity required?

Prescription Wording to Ask Your Doctor About

Your insurance company may require a prescription or letter of medical necessity from your doctor. You may ask your provider if they are comfortable using language such as:

“Cranial prosthesis for medical hair loss due to [diagnosis/condition]. HCPCS A9282. Medically necessary for the patient’s hair loss related to [condition/treatment].”

Your doctor should include the diagnosis or condition that applies to you. Locks and Tops does not diagnose medical conditions or determine insurance eligibility.
